2024 TN School for the Blind Summer Learning Camps

Thank you for your interest in summer camps at TSB. This year much of the application can be completed online at: https://forms.office.com/r/b9JwUHzV6X

 

There are two additional documents that require parent/guardian signatures and need to be submitted separately (pages 2 and 3 of this packet)

 

 

As in the past, there is medical documentation required for students to attend. There is a check list in the pack for this information. Medical forms can be found below on the "Forms" section of this page. This includes an area for specialty forms as well such as diabetes, asthma, seizure, etc.  For special medical needs, please complete the "All Care Plans" form.

 

A printable version of the camp application form is available below for those unable to complete it online.

 

No application will be considered complete until the online application (or printed application), two signed forms on pages 2 and 3, and all medical documentation are submitted and reviewed for completion.
